Terms & Conditions

Effective and last updated: September 9, 2026

Acceptance and Scope

These Terms and Conditions ("Terms") govern your access to and use of www.spinelabs.com and related webpages, online forms, appointment-request experiences, content, and digital services associated with the Spine Labs brand (collectively, the "Website"). By using the Website, you agree to these Terms and the Spine Labs Privacy Policy. If you do not agree, do not use the Website.

Independently Operated Locations

Spine Labs locations are independently owned and operated. The Website may help you locate, contact, or request an appointment with a participating Spine Labs location, but the applicable location is responsible for the professional services it provides, its personnel, clinical decisions, scheduling, payments, records, and location-specific policies. References to "Spine Labs" in these Terms describe the brand and Website generally and do not eliminate any location's independent legal status.

A location may provide additional terms, disclosures, consent forms, financial policies, privacy notices, or patient agreements. If those terms conflict with these Terms regarding services provided by that location, the location-specific terms will control to the extent of the conflict.

Website Information Is Not Medical Advice

Website content is provided for general education and information only. It is not medical advice, diagnosis, treatment, or a substitute for evaluation by a qualified healthcare professional. Using the Website, submitting a form, or communicating electronically does not, by itself, create a doctor-patient relationship. A professional relationship is established only as determined by the applicable clinic and applicable law.

Individual results vary. Statements about services, conditions, outcomes, or benefits are not guarantees. Always seek appropriate professional advice regarding a medical condition or treatment decision.

Emergencies

Do not use the Website, email, online forms, or text messaging for a medical emergency. Call 911 or seek immediate emergency care.

Appointments, Services, Pricing, and Offers

Appointment requests submitted through the Website are not confirmed until the applicable location accepts or confirms them. Availability, services, providers, hours, membership plans, prices, payment options, and promotional offers vary by location and may change.

Offers may be limited to qualifying new patients, local residents, specific services, time periods, or participating locations and may be subject to additional legal and clinical restrictions. Review the disclosures shown with the applicable offer and contact the location for complete details.

Forms and Information You Submit

You agree to provide accurate, current, and complete information and not to impersonate another person. You are responsible for ensuring that you are authorized to provide any telephone number, email address, or other information submitted through the Website. Do not submit unlawful, misleading, infringing, malicious, or harmful content.

Payments, Cancellations, and Refunds

Payments, membership terms, recurring charges, cancellation requirements, refund policies, and financial arrangements for clinical services are established by the applicable independently operated location and any agreement you enter into with that location. Contact the location directly with billing or cancellation questions.

SMS and MMS Program Terms

Program and sender. Spine Labs and participating independently owned and operated Spine Labs locations may send text messages through separate programs. Messages may include appointment confirmations, reminders, scheduling updates, service notifications, customer-support communications, promotions, special offers, discounts, and service updates, depending on the consent option (s) you select.

Consent. You must affirmatively opt in to receive messages. When we offer both marketing and non-marketing messages, we request consent separately. SMS consent is optional. Consent to receive marketing messages is not required to purchase goods or services, schedule an appointment, or receive care. By opting in, you confirm that you are the subscriber or customary user of the telephone number provided and are authorized to consent to receive messages at that number.

Age. The SMS program is intended for individuals who are at least 18 years old. A parent or legal guardian may opt in using their own telephone number for services requested for a minor, when legally permitted.

Frequency and charges. Message frequency varies. Message and data rates may apply, depending on your mobile service plan. Spine Labs and the applicable location do not charge a separate fee for the SMS program, but your carrier may charge you.

Opt out. You may cancel SMS messages from a sending program at any time by replying STOP. You may receive a final message confirming your opt-out. After that confirmation, you will no longer receive messages from that program unless you opt in again. Because Spine Labs locations may operate separate messaging programs, you may need to opt out separately from each sending number or program.

Help. Reply HELP for assistance or contact the applicable location through https://www.spinelabs.com/locations.

Delivery. Message delivery depends on effective transmission by your wireless carrier and is not guaranteed. Carriers are not liable for delayed or undelivered messages. Supported carriers and program availability may change.

Privacy. Information collected through the SMS program is handled as described in the Privacy Policy at https://www.spinelabs.com/privacy-policy and any applicable location-specific privacy notice.

Governing Law

These Terms and disputes concerning the Website are governed by the laws of the State of Florida, without regard to conflict-of-law principles, except where another law must apply. Agreements and disputes concerning clinical services, payments, or records may be governed by the terms and law applicable to the independently operated location involved.

Changes and Termination

We may update these Terms by posting a revised version with a new effective or last-updated date. Continued use of the Website after the changes take effect constitutes acceptance of the revised Terms. We may suspend or terminate access to the Website when reasonably necessary for security, maintenance, legal compliance, or misuse.

Severability and Entire Agreement

If any provision of these Terms is held unenforceable, the remaining provisions will remain in effect. These Terms, together with the Privacy Policy and any notices presented through the Website, constitute the agreement governing use of the Website. Location-specific agreements remain separately applicable to services provided by that location.
