Privacy Policy

Effective and last updated: September 9, 2026

Scope and Who We Are

This Privacy Policy explains how Spine Labs collects, uses, discloses, and protects information obtained through www.spinelabs.com, related webpages, online forms, appointment-request experiences, and communications associated with the Spine Labs brand (collectively, the "Website").

Spine Labs locations are independently owned and operated. When you select, contact, or receive services from a particular location, that location may collect and process your information as a separate business and healthcare provider. The applicable location may provide supplemental privacy notices or a Notice of Privacy Practices that governs information collected in connection with clinical care.

Information We Collect

Depending on how you interact with the Website or a Spine Labs location, we may collect:

Contact and identity information, such as your name, email address, mailing address, and telephone number.

Appointment and service-request information, such as your preferred location, requested service, preferred date and time, and communications with a Spine Labs team.

Information you provide in online forms, surveys, promotions, franchise inquiries, employment inquiries, or other communications.

SMS consent records, including the consent option selected, date and time, source page or form, disclosure version, telephone number, and opt-out history.

Website and device information, such as IP address, browser type, device identifiers, referring URL, pages viewed, interactions, and cookie or similar technology data.

Payment or transaction information when applicable. Third-party payment providers may process payment card information directly rather than Spine Labs storing it.

Other information you voluntarily provide. Please do not send sensitive medical information through ordinary text messages, email, or unsecured forms unless specifically requested through an appropriate channel.

How We Collect Information

We collect information directly from you when you complete a form, request or schedule an appointment, contact a location, subscribe to communications, apply for a franchise or position, make a payment, or otherwise interact with us. We may also collect information automatically through cookies and similar technologies and receive information from service providers that support website hosting, scheduling, communications, analytics, advertising, payment processing, and related business functions.

How We Use Information

We may use information to:

Operate, maintain, personalize, and improve the Website.

Connect you with the Spine Labs location you select or that is best suited to respond to your request.

Process appointment requests, inquiries, payments, and other transactions.

Provide customer service and respond to questions.

Send appointment confirmations, reminders, scheduling updates, service notifications, and support communications when authorized.

Send marketing or promotional communications when you have provided the required consent.

Measure website performance, advertising effectiveness, and user engagement.

Maintain security, prevent fraud or misuse, enforce applicable terms, and comply with legal obligations.

Cookies, Analytics, and Advertising

The Website may use cookies, pixels, tags, and similar technologies to remember preferences, understand Website usage, improve performance, and measure advertising. Some technologies may be provided by analytics, advertising, social-media, or other service providers. You may be able to limit cookies through your browser or device settings. Disabling cookies may affect Website functionality.

Where required by applicable law, we will request consent before using non-essential cookies or honor legally recognized browser-based opt-out signals.

How We Disclose Information

We may disclose information only as reasonably necessary for the purposes described in this Policy, including:

To the independently owned and operated Spine Labs location you select, contact, or ask to provide services.

To service providers that perform functions on our behalf, such as website hosting, scheduling, communications, customer support, analytics, advertising measurement, payment processing, and security.

When required by law, legal process, court order, or governmental request, or when reasonably necessary to protect rights, safety, property, or security.

In connection with a merger, acquisition, financing, reorganization, or sale of all or part of a business, subject to applicable law.

With your direction or consent.

SMS and MMS Communications

Spine Labs and participating independently owned and operated Spine Labs locations may offer separate text-messaging programs for non-marketing communications, such as appointment confirmations, reminders, scheduling updates, service notifications, and customer support, and for marketing communications, such as promotions, special offers, discounts, and service updates.

Text-message consent is collected through an affirmative, optional action. Marketing and non-marketing consent are requested separately when both types of messages are offered. Consent to receive marketing text messages is not a condition of purchasing goods or services, scheduling an appointment, or receiving care. Message frequency varies. Message and data rates may apply. Reply STOP to opt out of messages from the sending program. Reply HELP for assistance or contact the applicable Spine Labs location. An opt-out request applies to the applicable sending program and telephone number; you may need to opt out separately from another independently operated location or program.

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Carriers are not liable for delayed or undelivered messages. The SMS program is intended for individuals 18 years of age or older. A parent or legal guardian may provide contact information and consent in connection with services requested for a minor when legally permitted.

Health and Clinical Information

This general Website Privacy Policy is not a Notice of Privacy Practices under the Health Insurance Portability and Accountability Act (HIPAA). Information created or maintained by a Spine Labs location in connection with clinical care may be governed by that location's Notice of Privacy Practices and applicable health-privacy law. Contact the applicable location for information about its clinical privacy practices or to exercise rights relating to medical records.

Data Retention

We retain personal information for as long as reasonably necessary to fulfill the purposes described in this Policy, provide requested services, maintain consent and transaction records, comply with legal and regulatory obligations, resolve disputes, and enforce agreements. Retention periods vary depending on the type of information and the context in which it was collected.

Data Security

We use reasonable administrative, technical, and physical safeguards designed to protect personal information. No website, transmission method, or storage system is completely secure, and we cannot guarantee absolute security.

Your Choices and Privacy Rights

You may unsubscribe from marketing emails by using the unsubscribe link in the message and may opt out of text messages by replying STOP. You may also adjust browser cookie settings.

Depending on where you live and subject to applicable exceptions, you may have rights to request access to, correction of, deletion of, or a copy of certain personal information, or to appeal a decision concerning a privacy request. To submit a request, use the Contact page at https://www.spinelabs.com/contact or contact the applicable Spine Labs location. We may need to verify your identity before completing a request.

Children's Privacy

The Website is not directed to children under 13, and we do not knowingly collect personal information directly from children under 13 through the Website without appropriate authorization. A parent or legal guardian may provide information on behalf of a minor when requesting healthcare services.

Third-Party Websites and Services

The Website may contain links to third-party websites, platforms, or services. Their privacy practices are governed by their own policies, and we are not responsible for their content, security, or privacy practices.

Changes to This Policy

We may update this Privacy Policy periodically. We will post the revised version on this page with a new effective or last-updated date. We may communicate material changes through the Website or another appropriate method.
